On or about July 28, 2011, at the above addressed jobsite, employees conducting water installation in a trench six (6') feet to seven (7') feet three (3") inches in depth did not have a safe means of egress.    THE REICHELT PLUMBING, INC., WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.651(C)(2), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 310177621, CITATION NUMBER 1, ITEM NUMBER 1 AND WAS AFFIRMED AS A FINAL ORDER ON AUGUST 14, 2007, WITH RESPECT TO A WORKPLACE LOCATED AT 16250 WAUSAU AVE, SOUTH HOLLAND, IL 60473.

29 CFR 1926.652(a)(1):  Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section:    a. On or about July 28, 2011, at the above addressed jobsite, employees were installing a water line in a trench measured at six (6') feet to seven (7') feet three (3") inches in depth.  The trench was not provided with cave in protection, thereby exposing the employees to cave in hazards.
