29 CFR 1926.652(a)(1): Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: a. On or about August 31, 2011, at the above addressed jobsite, an employee was pressure testing a pipe in a trench measured at six (6') feet two (2") inches in depth. The trench was not provided with cave in protection, thereby exposing the employee to cave in hazards. THE L.A. SITES, INC., WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.652(a)(1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 312592868, CITATION NUMBER 2, ITEM NUMBER 2 AND WAS AFFIRMED AS A FINAL ORDER ON MARCH 12, 2010, WITH RESPECT TO A WORKPLACE LOCATED AT 1440 RTE. 59, JOLIET, IL 60431.
