29 CFR 1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered in operative. Production department - On or about September 27, 2011, the employer did not establish an energy control program for employees performing machine set up on the wet saw, thereby exposing employees to the hazard of unexpected energization of machinery.

29 CFR 1910.178(l)(1)(i): The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l). Throughout facility - On or about September 27, 2011, operators were not trained in the safe operation of powered industrial trucks, thereby exposing all employees in the facility to struck by hazards.

29 CFR 1910.178(q)(7): Industrial trucks shall be examined before being placed in service, and shall not be placed in service if the examination shows any condition adversely affecting the safety of the vehicle. Such examination shall be made at least daily. Where industrial trucks are used on a round-the-clock basis, they shall be examined after each shift. Defects when found shall be immediately reported and corrected. Throughout facility - On or about September 27, 2011, the employer did not perform an examination of the powered industrial equipment to check for defects, thereby exposing employees to serious struck by hazards.

29 CFR 1910.305(e)(1): Cabinets, cutout boxes, fittings, boxes, and panel board enclosures. Cabinets, cutout boxes, fittings, boxes, and panel board enclosures in damp or wet locations shall be installed so as to prevent moisture or water from entering and accumulating within the enclosures and shall be mounted so there is at least 6.35-mm (0.25-in.) airspace between the enclosure and the wall or other supporting surface. However, nonmetallic enclosures may be installed without the airspace on a concrete, masonry, tile, or similar surface. The enclosures shall be weatherproof in wet locations. Production department - On or about September 27, 2011, the employer had not enclosed the electrical cabinet to prevent moisture or water from entering or accumulating, thereby exposing employees to electrical hazards.
