29 CFR 1926.404(b)(1)(i): The employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites. These requirements are in addition to any other requirements for equipment grounding conductors: (a) Near the east side of the building on or about November 2, 2011, there was no assured equipment grounding conductor program or ground-fault circuit-interrupter protection when an extension cord was used to power a 110 VAC, double insulated DeWalt sawsaw. The sawsaw was used to cut 2x4s.

The sawsaw was used to cut 2x4s.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds were not fully planked or decked between the front uprights and the guardrail supports: (a) Near the east side of the building on or about November 2, 2011, a mobile scaffold working level was not fully planked as there were 2, 16-inch wide planks between the uprights that were 58 inches apart. Employees on the scaffold removed sign boards from the building and repaired the glass on building. The working level was 15 feet, 9 inches above the asphalt below.

Employees on the scaffold removed sign boards from the building and repaired the glass on building. The working level was 15 feet, 9 inches above the asphalt below.
