29 CFR 1910.1200(h)(1):     Employers shall provide employees with effective information and training on hazardous chemicals intheir work area at the time of their initial assignment, and whenever a new physical or health hazard the employees have not previously been trainedabout is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability,carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and material safety datasheets.    a) Employees had not been trained on the employer's written Hazard Communication Program regarding chemicals in the workplace such as but not limited to cutting and tapping fluid, cool cut (coolant), acetone, degreasers, and oils.

29 CFR 1910.132(d)(2):     'The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment.    a) A certified written hazard assessment had not been completed by the employer to determine what personal protective equipment (PPE) must be worn in specific areas and/or during certain activities such as but limited to grinding on stainless steel as well as handling cutting and tapping fluid, cool cut (grinding coolant), acetone, degreasers, and oils.

29 CFR 1910.134(k)(6):     The basic advisory information on respirators, as presented in Appendix D of this section, shall be provided by the employer in any written or oral format, to employees who wear respirators when such use is not required by this section or by the employer.    a) Employees were not provided a copy of the Appendix D located in the Respiratory Protection Standard for voluntary use of a Gerson 1501 Nuisance filtering facepiece (dusk mask).
