29 CFR 1926.403(I)(2)(i): Except as required or permitted else where in this subpart, live parts of electric equipment operating at 50 volts or more shall be guarded against accidental contact by cabinets or other forms of enclosures, or by any of the following means: On or about August 16, 2011, at the jobsite located at 9732 S. Merton Avenue, Oak Lawn, Illinois - employees were working in a rehab using receptacles and light swtiches that were not protected with covers or face plates to protect against accidental contact, thereby exposing employees to electrical hazards.

29 CFR 1926.404(b)(1)(i): The employer shall use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites. These requirements are in addition to any other requirements for equipment grounding conductors. On or about August 16, 2011, at the jobsite located at 9732 S. Merton Avenue, Oak Lawn, Illinois - employees were using electric tools such as, but not limited to, power drills and radios were not using ground faults circuit interrupters or did not have an assured equipment grounding program, thereby exposing employees to electrical hazards.

29 CFR 1926.405(b)(2): All pull boxes, junction boxes, and fittings shall be provided with covers. If metal covers are used, they shall be grounded. In energized installations each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear: On or about August 16, 2011, at the jobsite located at 9732 S. Merton Avenue, Oak Lawn, Illinois - employees were performing drywall activities in a rehab where the electrical panel box was not equipped with a cover, thereby exposing employees to electrical hazards.
