29 CFR 1910.23(b)(5): Where there is a hazard of materials falling through a wall hole, and the lower edge of the nearside of the hole is less than 4 inches above the floor, and the far side of the hole more than 5 feet above the next lower level, the hole shall beprotected by a standard toeboard, or an enclosing screen either of solid construction, or as specified in paragraph (e)(11) of this section. (a) At the stock room B of Bldg 2273, the mezzanine storage did not have a toe board.

29 CFR 1910.304(f)(1)(iv): Over current devices shall be readily accessible to each employee or authorized building management personnel. These over current devices may not be located where they will be exposed to physical damage or in the vicinity of easily ignitable material. (a) In the vehicle maintenance shop, the safety switch for the vent system was blocked by a cabinet. The safety switch was not readily accessible or capable of being reached quickly for operation.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws. (a) In the employee break room, the flexible power cord of the Bunn coffee maker did not have strain relief, the insulation was pulled out from the sheeting.
