29 CFR 1910.1200(e)(1):  Employers shall develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, material safety data sheets, and employee information and training will be met, and which also includes the following:    On or about September 6, 2011 at jobsite located at 7325 W. 79th Street, Bridgeview, IL - There was no written hazardous communication program developed to inform employees using Amerimix Mortar Type S of the hazards associated with chemicals such as, but not limited to Portland cement, thereby exposing the employees to health hazards.

29 CFR 1926.95(a):  Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:    On or about September 6, 2011 at jobsite located at 7325 W. 79th Street, Bridgeview, IL - Employees using Amerimix Mortar Type S, containing portland cement were not using appropriated personal protectinve equipment, including, but not limited to face mask, gloves, or long sleeved clothing, while mixing and handling this product for masonry work. The employees were thereby, exposed to health hazards.

29 CFR 1926.100(a):  Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns shall be protected by protective helmets:    On or about September 6, 2011 at jobsite located at 7325 W. 79th Street, Bridgeview, IL - An employee working below a tubular welded frame scaffold while brick removal was being performed above was not protected against falling meterails and debris by the use of  a hard hat, thereby exposing the employee to struck by hazards.
