29 CFR 1926.403(i)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by cabinets or other forms of enclosures, or by any of the following means: (A) by location in a room, vault, or similar enclosure that is accessible only to qualified persons; (B) by partitions or screens so arranged that only qualified persons will have access to the space within reach of the live parts; (C) by location on a balcony, gallery, or platform so elevated and arranged as to exclude unqualified persons, (D) by elevation of 8 feet or more above the floor or other working surfaces and so installed as to exclude unqualified persons: a) On or about October 7, 2011, at the jobsite located at 5921 S. Wabash Avenue, Chicago, IL - employees and subcontractors employees were using receptacles and light switches that were not provided with covers or face plates to protect against accidental contact, thereby exposing employees to electrical hazards.

29 CFR 1926.405(b)(1): Conductors entering boxes, cabinets, or fittings. Conductors entering boxes, cabinets, or fittings will be protected from abrasion, and openings through which conductors enter shall be effectively closed. Unused openings in cabinets, boxes, and fittings shall also be effectively closed: a) On or about October 7, 2011, at the jobsite located at 5921 S. Wabash Avenue, Chicago, IL - employees performing electrical activities in a rehab were exposed to electrical hazards due to an electric cord laying across the open metal frame of the panel box and was not protect against accidental abrasion.
