29 CFR 1910.134(c)(1): In any workplace where respirators are necessary to protect the health of the employee or whenever respirators are required by the employer, the employer shall establish and implement a written respiratory protection program with worksite-specific procedures. The program shall be updated as necessary to reflect those changes in workplace conditions that affect respirator use. The employer shall include in the program the following provisions of this section, as applicable: a) On October 7, 2011 - at the jobsite located at 5921 S. Wabash Avenue, Chicago, IL - the employer did not implementa written respirator program for employees using respirators while spraying foam insulation in a rehab, employees were thereby exposed to health hazards.

29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact: a) On October 7, 2011 - at the jobsite located at 5921 S. Wabash Avenue, Chicago, IL - an employee performing insulation clean-up was not using proper protective equipment, including but not limited to eye glasses, face shield, or a respirator.

Flexible cords and cables may pass through doorways or other pinch points, if protection is provided to avoid damage. a) On October 7, 2011 - at the jobsite located at 5921 S. Wabash Avenue, Chicago, IL - employees performing insulation activities in a rehab were exposed to electrical hazards due to: 1) electrical extension cords running through doorways 2) the insulation machine cord laying across the open metal frame of the panel box Neither were protected against accidental damage and/or abrasions.

Wabash Avenue, Chicago, IL - employees performing insulation activities in a rehab on the third floor were using receptacle and light switches that were not provided with covers or face plates to protect against accidental contact, thereby exposing the employees to electrical hazards.
