29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: M-1 Tool Works, Inc., did not develop and implement a written lockout program. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d)

29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed: Maintenance employees who perform voltage testing on machinery, were not supplied with nor were required to wear appropriate personal protective equipment. Abatement certification is required for this item in accordance with 29 CFR 1904.19(c)
