29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: Instance a. On or about October 11-18, 2011, employees were exposed to the unprotected excavation walls which were located on the west side of the job site. Employees were exposed while constructing the block retaining wall near the KUA tennis court. Instance b. On or about August 11, 2011, employees were exposed to unprotected excavation walls which were located on the east side of the job site. Employees were exposed while installing the lean concrete and installing formwork on top of the lean concrete. Instance c. On or about October 7, 2011, employees were exposed to the unprotected excavation walls which were located on the west side of the job site. Employees were exposed while performing steel erection at the west wall near the KUA tennis court.
