29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms and service room were not kept clean and orderly and in a sanitary condition: Employees were exposed to slipping and tripping injuries because the shop floor was not kept in a clean and orderly condition: a) Directly outside the office door and adjacent to the extension ladder giving access to files above the office was a loose pile of papers, cabinet parts and binders. b) In the back of the shop, surrounded by wood racking, the spring stock lay loose on the floor.

29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toe boards: Employees accessing mezzanine levels were exposed to fall hazards because the employer had not provided guardrails around the perimeter of the mezzanine level: a) Employees accessed a mezzanine level above the reception area approximately nine feet high to retrieve files and there were no guardrails. b) Employees accessed a mezzanine level above the back offices approximately nine feet high to retrieve parts and there were no guardrails.
