CFR 1926.100(a):  Employer did not provide head protection for employees working in areas where there was a possible danger of head injury from impact, or from falling or flying objects.    	(a)	SMA Framing, 2823 North Ave., Grand Junction, CO 81501:  On and before November 3, 2011, the exposing employer failed to ensure that adequate head protection was provided for employees working in areas where there was a possible danger of head injury.  Specifically, an employee was witnessed working on a second floor balcony without a hard while work with nail guns and hand tools was taking place above him.  Condition exposed employee to the possibility of head injury.

(a)	SMA Framing, 2823 North Ave., Grand Junction, CO 81501:  On and before November 3, 2011, the exposing employer failed to ensure that adequate eye and face protection was provided for employees working in areas where there was a possible danger of eye injury.  Specifically, an employee was witnessed working on a second floor balcony without adequate eye protection while work with nail guns and hand tools was taking place above him.  Condition exposed employee to the possibility of eye injury.

CFR 1926.501(b) (13):  Each employee engaged residential construction activities 6 feet  (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems;    	(a)	SMA Framing, 2823 North Ave., Grand Junction, CO 81501:  On and before November 3, 2011, as the exposing employer, six employees working on a flat roof located at the site listed above were not protected from falling by a system as described above.  Specifically, three employees were witnessed utilizing nail guns while leaning over the edge of the roof, while the other three employees performed other types of work such as laying down 3/4", 4' x 8' sheets of plywood to deck the roof.  Condition exposed employees to fall hazards of over 30 feet.                    (b)	SMA Framing, 2823 North Ave., Grand Junction, CO 81501:  On and before November 3, 2011, as the exposing employer, an employee was witnessed working on a second floor balcony without guard rails or other fall protective measures in place to provide fall protection.  Condition exposed employee to fall hazards of over 20 feet.
