29 CFR 1910.305(a)(2)(x):     Flexible cords and cables shall be protected from accidental damage, as might be caused, for example, by sharp corners, projections, and doorways or other pinch points.    a)  East Bay 2 :  On August 1, 2011, and at times prior thereto, a flexible cord was laying across the aisle unprotected near the POL storage #6 & #7 and the Special Tool holding area creating a hazard.

29 CFR 1910.157(b)(1):     Where the employer has established and implemented a written fire safety policy which requires the immediate and total evacuation of employees from the workplace upon the sounding of a fire alarm signal and which includes an emergency action plan and a fire prevention plan which meet the requirements of 29 CFR 1910.38 and 29 CFR 1910.39 respectively, and when extinguishers are not available in the workplace, the employer is exempt from all requirements of this section unless a specific standard in part 1910 requires that a portable fire extinguisher be provided.      a)  Bays 1-4:  On or about August 1, 2011, and at times prior thereto, the Emergency  Fire Evacuation Maps posted did not show current changes in the evacuation routes and assembly areas for meeting points 1 & 2.

29 CFR 1910.305(b)(1)(ii):     Unused openings in cabinets, boxes, and fittings shall be effectively closed.    a)  West:  Mezzanine Level - Lunch Room:  On or about August 1, 2011, and at times prior thereto, there was a  4 square unmounted metal box with a loosened knock out on the underside located on top of a wooden cabinet.

29 CFR 1910.305(g)(1)(iv)(A):     As a substitute for the fixed wiring of a structure;    East-Bay 2 :  On August 1, 2011, and at times prior thereto,  a  flexible cord was used  in place of  fixed wiring  from the outlet  above the flammable storage cabinet # 6 running across the floor through a hole in the rear of the desk.

Electric equipment did not have markings giving voltage, current, wattage, or other ratings as necessary showing manufacturer's name or trademark.      Bay 2: On August 1, 2011, and at times prior thereto, a  flexible cord was used  in place of  fixed wiring  from the outlet  above the flammable storage cabinet # 6 running across the floor through a hole in the rear of the desk.  The flexible cord was plugged into a 4 square unmounted metal box placed on a metal trash can with plastic liner under the desktop creating an electrical and fire hazard .
