29 CFR 1926.501(b)(1):  Each employee on a walking/working surface having an unprotected side or edge which was six feet (1.8m) or more above a lower level was not protected from falling by the use of a guardrail system, a safety net system, or personal fall arrest system.    (a)C&E Framing LLC at 10425 E. 31st Ave., Denver, CO 80238:  On November 4, 2011 the employer did not ensure each employee on a walking/working surface having an unprotected side or edge which was six feet (1.8m) or more above a lower level was protected from falling by the use of a guardrail system, a safety net system, or personal fall arrest system in that an employee was engaged in decking operations on the south end of the floor without fall protection.  This condition exposed the employee to a fall hazard of approximately 11 feet.    (b)C&E Framing LLC at 10425 E. 31st Ave., Denver, CO 80238:  On November 4, 2011 the employer did not ensure each employee on a walking/working surface having an unprotected side or edge which was six feet (1.8m) or more above a lower level was protected from falling by the use of a guardrail system, a safety net system, or personal fall arrest system in that the employees engaged in decking operations were exposed to a fall hazard on the north end as they approached the ladder to exit the floor.  This condition exposed the employee to a fall hazard of approximately 11 feet.

29 CFR 1926.1053(b)(1): A portable ladder was used for access to an upper landing, and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing, and was not secured:    (a)C&E Faming LLC at 10425 E. 31st Ave., Denver, CO 80238:  On November 4, 2011 the employer did not ensure that a ladder, used to access the 2nd floor had side rails extending 3 feet above the upper landing, or was secured. This condition exposed the employees to uncontrolled fall hazards of approximately 11 feet.

29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person:     (a)  C&E Framing LLC at 10425 E. 31st Ave., Denver, CO 80238:  On November 4, 2011 the employer did not ensure the anchor used on the second story floor was designed and installed to maintain a safety factor of at least two.  Two employees were connected to one anchor and the employer could not verify the capabilities of the anchor as installed.  This condition may have exposed the employees to a fall hazard of approximately 11 feet.
