The company did not trained their employees to recognize, avoid, and prevent unsafe conditions while working inside the electrical substation.

29 CFR 1926.416(a)(1):     No employer shall permit an employee to work in such proximity to any part of an electric power circuit that the employee could contact the electric power circuit in the course of work, unless the employee is protected against electric shock by deenergizing the circuit and grounding it or by guarding it effectively by insulation or other means.    a)  Substation B Street, Beale AFB - On or about October 17, 2011 and at times prior thereto, three employees were permitted to work in proximity below a 60,000 voltage buss bar. The employees were bending a 20 foot rebar steel, creating an approximately 2 foot bend.  The rebar steel got stuck on the bender, an employee stood the rebar steel upright with the bend part of the rebar still inside the bender.  As the employee, who was holding the rebar steel stood it upright, electricity arced from the buss bar to the rebar.  The employee received burns on 50% of his body.
