(a) Star of the Sea Chapel (foyer entrance and adjacent rooms): On or about 11/1/2011, and times prior thereto, employees were working in an area where the lighting was hanging from the ceiling and not protected from accidental contact or breakage; exposing employees to electrical and broken glass hazards. (b) Star of the Sea Chapel (Bathroom): On or about 11/1/2011, and times prior thereto, employees were working in the bathroom area where an overhead bulb was not covered to protect it from accidental contact or breakage; exposing employees to electrical and broken glass hazards.

29 CFR 1926.405(a)(2)(ii)(F): Temporary lights shall not be suspended by their electric cords unless cords and lights are designed for this means of suspension. Star of the Sea Chapel (Foyer Entrance and adjacent Rooms): On or about 11/1/2011, and times prior thereto, employees were working in the foyer area where temporary lighting fixtures were hung by their electrical wiring; exposing employees to electrical hazards in the event that equipment came into contact with the wiring.

Star of the Sea Chapel (hallway adjacent to bathroom): On or about 11/1/2011, and times prior thereto, where employees were utilizing the breaker panel for powering the facility; the panel was missing blanks over circuits # 35, 37, 39, and 41 exposing employees to electrical hazards.

29 CFR 1926.416(a)(1): No employer shall permit an employee to work in such proximity to any part of an electric powercircuit that the employee could contact the electric power circuit in the course of work, unless the employee is protected against electric shock by deenergizing the circuit and grounding it or by guarding it effectively by insulation or other means. (a) Star of the Sea Chapel (Foyer area and adjacent rooms): On or about 11/1/2011, employees were engaged in work activities where open electrical circuits (unfinished light switches) were live and not protected or insulated to prevent accidental employee contact; exposing employees to electrical hazards. (b) Star of the Sea Chapel (Upstairs Choir Loft): On or about 11/1/2011, employees were engaged in work activities where an open electrical circuit (wiring from the ceiling) was hanging down without insulation or other means to prevent accidental employee contact; exposing employees to electrical hazards.
