Billiken Theatre, Projection Room:  On or about November 3, 2011 and at times prior thereto, the fire extinguisher mounted adjacent to the emergency exit showed a last inspection date of "8/11."  This condition exposed employees to smoke inhalaltion hazards in the event the fire extinguisher malfunctioned.

Sufficient access and working space shall be provided and maintained about all electric equipment to permit ready and safe operation and maintenance of such equipment.    (a)  Billiken Theatre, Projection Room:  On or about November 3, 2011, sufficient working space in front of Panel H was not provided.  This condition exposed employees to significant electrical shock and burn hazards from the 240/120 volt service.    (b)  Billiken Theatre, Projection Room:  On or about November 3, 2011, sufficient working space in front of Panel M was not provided.  This condition exposed employees to significant electrical shock and burn hazards from the 240/120 volt service.    (c) Billiken Theater, Refreshment Sales Area:  The electrical panel annotated as Panel MDP, in the closet behind the refreshment counter had materials stored in front of the panel, impeding access in the event of an emergency; exposing employees to electrical shock hazards in the event of an electrical emergency.
