Electric equipment shall be free from recognized hazards that are likely to cause death or serious physical harm to employees: Billiken Theater (Stage Area): On the right and left sides of the stage at the floor level, there were 2 energized bus bars exposed where the fuses had been pulled out and left open; exposing employees to electrical shock hazards in the event of an employee opening the cover. Billiken Theater (Stage Area): On the left side of the stage against the wall, an old electrical panel with open electrical lugs was in the workplace; exposing employees to electrical hazards in the event of an employee opening the cover.

Billiken Theater (Refreshment Sales Area): On or about 11/3/2011, and times prior thereto, employees were accessing and utilizing the electrical breaker panel for lighting and other electrical appliances without a cover over breaker #30; exposing employees to electrical shock hazards.

29 CFR 1910.305(g)(1)(iv)(C): Flexible cords and cables cannot be used where run through doorways, windows, or similar openings. Billiken Theater (Refreshment Sales Area): On or about 11/3/2011, a flexible cord was run through an opening on the counter of the refreshment stand resulting in damage to the electrical wiring; exposing employees to electrical shock hazards in the event of the ground becoming dislodged or the insulation becoming torn.
