29 CFR 1926.452(w)(2): Scaffold casters and wheels were not locked with positive wheel and/or wheel and swivel locks, or equivalent means, to prevent movement of the scaffold while the scaffold was used in a stationary manner: a) On or about November 7, 2011, while on a scaffold, employees moved it from one location to another by jogging it and rolling on the unlocked caster wheels.

29 CFR 1926.454(a): The employer did not have each employee who performs work while on a scaffold(s) trained by a person qualified in the subject matter to recognize those hazards associated with the type of scaffold(s) being used and to understand the procedures to control or minimize those hazards: a) On or about November 7, 2011, employees were not trained on hazards of mobile scaffolds in that, while on a scaffold, the employees moved it from one location to another by jogging it and rolling on the unlocked caster wheels.
