29 CFR 1910.147(c)(1):     The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    a) The employer had not established an Energy Control Program for the employees who performed service and maintenance to machines and printing equipment with in the facility.

29 CFR 1910.212(a)(1):     Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by ingoing nip points:    a) On or about November 2, 2011, the GBC laminator, model 6036-1, located in the Binding Department did not have a fixed guard installed to protect against accidental contact with the in-going nip points of the drive roll cylinders.

29 CFR 1910.219(f)(3):     Sprocket wheels and chains which were seven -7 feet or less above floors or platforms were not enclosed:    a) On or about November 2, 2011, the GBC laminator, model 6036-1, located in the Binding Department did not have a guard installed to protect contact with the drive chain and sprocket that powered the roll cylinders located approximately 32 inches off the floor.

29 CFR 1910.303(g)(2)(i):     Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):    a) On or about November 9, 2011 the 230 AC Volt rated electric GBR Laminator had exposed live electrical parts in that the machine was missing a side cover that would have protected all the electrical equipment from accidental contact by the operator.
