Abatement Note:  Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").    The employer shall assure that employees who decline to accept hepatitis B vaccination offered by the employer sign the statement in Appendix A.

29 CFR 1910.1030(g)(2)(vii)[F]: The training program did not contain an explanation of the use and limitations of methods that will prevent or reduce exposure including appropriate engineering controls, work practices, and personal protective equipment:    (a) 	Longmont Assisted Living Limited Partnership dba The Bridge Assisted Living at Life Care Center of Longmont, 2444 Pratt St., Longmont, CO 80501: On and before November 8, 2011 employees' bloodborne pathogen exposure control plan training did not include specific information on the contact time required to effectively disinfect surfaces for each disinfectant used at the facility.  Employees were not aware of the contact time for Oasis 499, the primary disinfectant used at the facility.  This condition exposed employees to a bloodborne pathogens hazard.
