The path to ground from circuits, equipment, and enclosures shall be permanent, continuous, and effective.    a) On 11/09/11 at the workplace, in the area behind the Roebel station, a Fasco Industries commercial duty circulator, was ungrounded causing the unit's casing to become electrically live. The grounding pin had been bent and broken loose inside the plug's housing preventing a continuous path.
