29 CFR 1903.19(c)(1):  Within 10 calendar days after the abatement date, the employer did not certify to OSHA that the cited violation had been abated:       On December 28, 2011, Twin Lakes Roof Systems Inc., was issued citations and the employer did not verify, through written certification that each citation was abated.  Abatement certification was not received for the following violations:      a.	Citation 1 Item 1a, and    b.	Citation 1 Item 1b.

29 CFR 1910.178(l)(1)(i):     The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):    Prior to allowing employees to use the powered industrial vehicle, the employer did not ensure that each operator had completed training that included all of the truck and workplace related topics listed in the standard and completed an evaluation of the operators performance through practical exercises.

29 CFR 1910.178(l)(6):      The employer did not certify that each operator has been trained and evaluated as required by paragraph (l) and shall include the name of the operator, the date of training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation:    The employer did not document the required powered industrial truck training and evaluation for each operator.  There was no written documentation that included the operator's name, date of the training, date of the evaluation and/or the name of the person conducting the evaluation.
