29 CFR 1926.404(b)(1)(iii)(C): Where an assured equipment grounding conductor program was being utilized, equipment found to be damaged or defective was being used prior to being repaired: (a) Cell House - East and West side Roof: On or about October 24, 2011 and at times prior thereto, the electric extension cord had damaged insulation exposing employees to an electrical hazard. The extension cord was to provided power to the 110 VAC Toro Ultra Blower.

The path to ground from circuits, equipment, and enclosures were not permanent and continuous. a) Cell House - East and West side of Roof: On October 24, 2011 and at times prior thereto, on the project, the ground pin was missing on an extension cord, exposing employees to hazards who used the cord to power a 110 VAC Toro Ultra Blower used for roofing work.
