29 CFR 1926.404(b)(1)(iii)(C): Where an assured equipment grounding conductor program was being utilized, equipment found to be damaged or defective was being used prior to being repaired: (a) Cell House East and West side of Roof: On or about October 24, 2011 and at times prior thereto, the electric extension cord used had damaged insulation exposing employees to an electrical hazard. The extension cord was used to provide power to the 110 VAC Toro Ultra Blower.

The path to ground from circuits, equipment, and enclosures were not permanent and continuous. a) Cell House- East and West side of Roof: On or about October 24, 2011 and at times prior thereto, on the project, the ground pin was missing on the extension cord being used exposing employees to an electrical hazard. The extension cord was used to supply power to the 110 VAC Toro Ultra Blower.
