Covers and/or guardrails were not provided to protect personnel from the hazards of open pits, tanks, vats, ditches, etc. a) Island Choice, Generator Room Floor, north side: On or about 30 AUG 11, several metal plates on the floor were missing and leaving openings on the floor and over the water drainage. One metal plate was badly rusted leaving holes and creating a tripping and falling hazardls.

29 CFR 1910.305(g)(1)(iv)(C): The electrical extension cord ran through doorways, windows, or similar openings; a) Island Choice, cooling tower: On or about 30 AUG 11, a yellow extension cord to the three portable fans was lying on the floor and running to the back end of the cooling tower. The cord was exposed to foot traffic exposing workers in the area to contact electricity. The electrical cord was also spliced and tap in several areas creating a shock hazard.
