29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided: a) The employees performing tuckpointing at 509 W. Wisconsin Avenue Milwaukee, WI 53203 were not provided stairs or other means to access the scaffold other than crawling over a parapet more than 32 inches high and 21 1/2 inches wide with tools and equipment on the roof.

29 CFR 1910.134(g)(1)(i)(A): Respirators with tight-fitting facepieces were worn by employees who had facial hair that came between the sealing surface of the facepiece and the face or that interfered with valve function: (Construction Reference 1926.103) a) The forman/employee used a 3M full facepiece elastomeric tight fitting respirator performing tuck point work with a beard and facial hair that interferes with the seal of the respirator. b) The general laborer employee used a 3M full facepiece elastomeric tight fitting respirator performing tuck point work with a beard and facial hair that interferes with the seal of the respirator.
