29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, or other approved alternative fall protection system: (a) Rodriguez Framing at 2203 & 2209 Cocklebur Lane, Fort Collins, Colorado 80525: On or before November 7, 2011 the employer did not ensure the employees were protected from injury, in that employees engaged in residential construction activities on a roof with a fall hazard of approximately 10-11 feet without the use of an adequate fall protection system.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards: (a) Rodriguez Framing at 2203 & 2209 Cocklebur Lane, Fort Collins, Colorado 80525: On or before November 7, 2011 the employer did not ensure that each employee engaged in residential construction activities were trained to recognize the hazards of working at elevation and trained in the use of fall protection. Employees engaged in residential construction activities on a roof worked near the edge of a 4 in 12 pitch roof on a residential home without the use of a fall protection system. This condition exposed employees to a fall of approximately 10-11 feet.
