Operators could be caught by the Skagit winch's unguarded rotating parts, which were not tagged or otherwise render inoperable.

29 CFR 1926.20(b)(4): The employer did not ensure that the employee(s) required to operate equipment and machinery were qualified by training or experience. CHS- Kalama, WA.- (a) Operator(s) of the spud winch, located on the vessel "Sea Hawk" were not fully aware of the clutch pedal used to shift the transmission and/or bring the equipments rotating parts to a stand still. (b) Maintenance personnel failed to identify the spud winch unsafe or render it inoperable for continued use untill the equipments guards were reinstalled.
