29 CFR 1910.147(c)(4)(i):     Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:      a) On or about November 14, 2011, the employer had not established Energy Control Procedures for the employees who performed servicing and maintenance on equipment with in the facility. The employer did not have procedures to isolate and lockout the energy sources to boiler equipment such as the hot water supply line(s) and the electric disconnect.

29 CFR 1910.303(f)(2):     Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, was not legibly marked to indicate its purpose, nor located and arranged so the purpose was evident:    a) On or about November 14, 2011, the 200-Amp feeder electric breaker panel located outside the Boiler Room and the wall-mounted branch circuit breaker panel located inside the boiler room did not have the over current breaker devices in each panel labeled to identify to its service location.

29 CFR 1910.332(b)(1):     Employees were not trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertained to their respective job assignments:    a) On or about the November 14, 2011, an employee was working on electrical equipment at the facility that was an energized 208 AC volt 3 Phase circuit located in the boiler room without properly isolating the power fromo the equipment. The employee was exposed to electrical hazards but was not instructed in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to assist in the control or elimination of any hazards or other exposure to illness or injury.  The employee was not trained in and familiar with the potential hazards to which he was exposed nor any electrically related safety practices not specifically addressed by 29 CFR 1910.331 through 1910.335 but which were necessary for the employee's safety.
