29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around each hole: Schott Construction LLC, at 825 S Adams Street, Denver, CO: On or about 11/14/11, Schott Construction did not ensure that each employee on walking/working surfaces was protected from falling through a hole more than 6 feet (1.8 m) above the lower level, by personal fall arrest systems, covers, or guardrail systems erected around the hole. An employee of Schott Construction was installing a patio door and its flashing adjacent to a large window well. The approximate depth of the window well was six feet nine inches. The employee was not protected from falling into the hole by a guardrail system erected around the hole. This condition exposed the employee to an approximate six foot nine inch fall hazard.
