29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) On or about November 16, 2011 and at times prior thereto, employees were exposed to amputation hazards while using an unguarded, four foot hydraulic press brake to bend material. b) On or about November 16, 2011 and at times prior thereto, employees were exposed to amputation hazards while using the only pathway through Shop #3, which led to the back of an unguarded hydraulic press brake.
