29 CFR 1926.95(a): Protective equipment, including personal protective equipment for the eyes, was not used where ever it was necessary by reason of hazards encountered in a manner capable of causing injury or impairment in the function of any part of the body through physical contact: a) Steury Enterprises, LLC, Perrysburg, Ohio: On or about November 16, 2011, at address 28370 Kensington Lane, Perrysburg, Ohio, the employer failed to ensure that each employee used appropriate eye protection while engaged in nailing activities, such as securing shingles to the structure roof with a pneumatic nailer, exposing employees to eye injuries from flying particles.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels were not protected from falling by guardrail systems with toebooards, safety net systems, or personal fall arrest systems: a) Steury Enterprises, LLC, Perrysburg, Ohio: On or about November 16, 2011, at address 28370 Kensington Lane, Perrysburg, Ohio, the employer did not ensure that employees were protected from falls while applying new roofing to a steep sloped roof (6:12) on a commercial type structure. Employees were exposed to potential falls up to 13.6 feet.
