29 CFR 1910.23(a)(8)(ii):  A floor hole into which persons can accidentally walk into was not guarded by a floor hole cover of standard strength and construction:    a) On or about October 30, 2011 and times prior, Cirque Resources LP, as controlling/exposing employer, did not ensure that DHS Drilling Company, Inc, a subcontractor, protected their employees from injury in that, half of the cellar floor cover was missing. The opening was 4 foot by 2 foot.
