29 CFR 1926.100(a):     Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:    S & J Residential Roofing, conducting work at 440 South Coy Rd., Oregon, OH: On or about November 17, 2011, the employer did not ensure that employees were protected from head injury hazards by protective hard hats while working below employees who were replacing a low slope roof of the one story commercial building. Employees were exposed to potential impact injuries caused by falling materials and/or equipment.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1926.501(b)(10):     Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems:    S & J Residential Roofing, conducting work at 440 South Coy Rd., Oregon, OH: On or about November 17, 2011, the employer did not ensure that employees were protected from fall hazards while replacing a low slope roof of the one story commercial building. Employees were exposed to approximately a 12 foot fall hazard from the roof to the ground below.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1926.503(a)(1):     The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    S & J Residential Roofing, conducting work at 440 South Coy Rd., Oregon, OH: On or about November 17, 2011, the employer failed to ensure that employees had been trained in the appropriate use of fall protection such as, but not limited to, the use of personal fall arrest systems, or alternative measures of fall protection while replacing a low slope roof of the one story commercial building. Employees did not use fall protection and were exposed to approximately a 12 foot fall hazard from the roof to the ground below.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1926.1053(b)(1):     Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    S & J Residential Roofing, conducting work at 440 South Coy Rd., Oregon, OH: On or about November 17, 2011, the employer did not ensure that an extension ladder, used to access the roof the one story commercial building, was extended at least 3 feet above the roof edge. The employees were exposed to approximately a 12 foot fall hazard when using the ladder to access the working level in order to conduct roofing activities.    No additional abatement information is needed for this item.
