Front St. in Perrysburg, OH: The employer did not assure that a personal fall arrest system was used at all times by employee(s) who were applying mortar to the structure.  Employees worked at an estimated 50 foot height from a Genie (model Z-135) aerial lift.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
