29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift: a) The Thomas Seiple Co. on or about November 18, 2011, at the worksite located at 215 E. Front St. in Perrysburg, OH: The employer did not assure that a personal fall arrest system was used at all times by employee(s) who were applying mortar to the structure. Employees worked at an estimated 50 foot height from a Genie (model Z-135) aerial lift. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
