29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8m) or more above lower levels were not protected by guardrail systems, safety net system, personal fall arrest system, or other approved alternative fall protection system: (a) Excellence in Building @ 3136 Hudson Drive, Loveland, CO 80537: On and before November 14th, 2011, the employer did not ensure that fall protection systems were utilized by employees engaged in roofing activities while working at the unprotected edges of a roof on a single-family home. Employees were not utilizing a fall protective system. The condition exposed employees to a fall hazard of approximately 10 feet.

29 CFR 1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid support, and a grasping device, such as a grabrail, was not provided to assist the employees in mounting and dismounting the ladder (a) Excellence In Building @ 3136 Hudson Drive, Loveland, CO 80537: On and before November 14th, 2011, the controlling employer did not ensure that employees were utilizing portable ladders in such a way that enabled them to access and egress from work areas using ladders that extended 3' above the walking working surface. Employees were using a heavy duty portable extension-ladder for access to the roof. Employees were exposed to a fall of approximately 10' while attempting to use the ladder.
