29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level: On or about 11/17/11 during construction activities including but not limited to working from a mobile scaffold, the employer did not ensure employees were protected from fall hazards approximately 12 feet. Employees were observed climbing the guardrails to access a roof to install plywood sheathing and rafters with no fall protection system.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and fuardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: a. On or about 11/17/2011, during construction activities including but not limited to roofing activities, the employer did not ensure employees were protected from fall hazards approximately 30 feet by ensuring a fall protection system system/device was being used. b. On or about 11/17/2011, during construction activities including but not limited to roofing activities, the employer did not ensure employees were protected from fall hazards approximately 15 feet by ensuring a fall protection system system/device was being used. c. On or about 11/17/2011, during construction activities including but not limited to roofing and carpentry activities, the employer did not ensure employees were protected from fall hazards approximately 12.3 feet by ensuring a fall protection system system/device was being used.
