29 CFR 1910.147(c)(4)(i):  Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in the activities covered by this section:    a)  On or about November 10, 2011, and at times prior, employees were exposed to caught-in hazards and amputations because the employer did not develop adequate energy control procedures and ensure that they were understood and used by employees.  Energy isolating devices were not utilized by all employees who performed hazardous tasks such as changing blades on the large table saws.

29 CFR 1910.213(h)(1):  The sides of the lower exposed portion of the blade were not guarded to the full diameter of the blade by a device that will automatically adjust itself to the thickness of the stock and remained in contact with stock being cut to give maximum protection possible for the operation being performed:    a)  On or about November 10, 2011, and at times prior, employees were exposed to lacerations and amputations while they operated the Ezy-Mitre radial saw (panel saw) in the shop area because the saw did not have a guard which covered the lower half of the blade.

The guard for the portion of the blade between the sliding guide and the upper-saw-wheel guard did not protect the saw blade at the front and outer side:    a)  On or about November 10, 2011, and at times prior, employees were exposed to lacerations and amputations while working with a bandsaw that was not adequately guarded above the guide rolls.

Without such information, employees were exposed to potential overexposure to the chemicals and/or hazardous reactions between incompatible chemicals.
