29 CFR 1910.132(f)(1): The employer shall provide training to each employee who is required by this section to use PPE: On or about 11/14/2011, training on fall protection equipment was not provided to maintenance employees who perform work at heights greater than 4 feet, including but not limited to work from aerial lifts. Training shall include the following: 1. When PPE is necessary; 2. What PPE is necessary; 3. How to properly don, doff, adjust, and wear PPE; 4. The limitations of the PPE; and, 5. The proper care, maintenance, useful life, and disposal of the PPE.

29 CFR 1910.178(p)(1): If at any time a powered industrial truck is found to be in need of repair, defective, or in any way unsafe, the truck shall be taken out of service until it has been restored to safe operating condition: On or about 11/14/2011, during the walkthrough part of the OSHA inspection, the #33 Hyster propane lift truck was observed in operation without the propane strap secured.

29 CFR 1910.179(n)(4)(i): At the beginning of each operator's shift, the upper limit switch of each hoist shall be tried out under no load. Extreme care shall be exercised; the block shall be "inched" into the limit or run in at slow speed. If the switch does not operate properly, the appointed person shall be immediately notified; On or about 11/14/2011, at least one operator of the P&H cranes in the forge area did not test the hoist upper limit switch at the beginning of the shift.
