29 CFR 1910.38(c)(3):   Minimum elements of an emergency action plan. An emergency action plan must include at a minimum:   Procedures to be followed by employees who remain to operate critical plant operations before they evacuate;      On or about November 21, 2011 an explosion and fire occurred in Building 401A and the evacuation plan was not adequate in that:    a) As required by 1910.120 (q)(1), procedures had not been developed for Turbo Air Operators located in Building 401 control room to follow when required to remain in the control room until the Test Cells were shut down before operators could evacuate;    b) Emergency shutdown procedures had not been updated for equipment such as but not limited to compressors,  pignones and boosters located in Bldg 401 and 401A after the installation of computers in the control room.

29 CFR 1910.38(f)(2):     Review of emergency action plan. An employer must review the emergency action plan with each employee covered by the plan when the employee's responsibilities under the plan change;     The employer did not ensure that the plan was reviewed with Turbo Air operators in Building 401 when the plan changed from employees evacuating to employees shutting down operations and performing emergency response prior to evacuation in that on or about November 21, 2011 employees remained in the control room until the test cell was shut down even though an evacuation order by the fire department had been initiated.
