29 CFR 1904.29(a):  The employer did not utilize OSHA 300, 300-A, and 301 forms, or equivalent forms, for recordable injuries and illnesses.     a) On or about October 12, 2011, in the workplace:  The employer did not complete the OSHA 300 form for workplace  injuries and illnesses that occurred in 2010.    b) On or about October 12, 2011, in the workplace:  The employer did not complete the OSHA 300-A form for workplace injuries and illnesses the occurred in 2010.    c) On or about October 12, 2011, in the workplace:  The employer did not complete the OSHA 300-A form for workplace injuries and illnesses the occurred in 2009.    d) On or about October 12, 2011, in the workplace:  The employer did not complete the OSHA 300-A form for workplace injuries and illnesses the occurred in 2008.    e) On or about October 12, 2011, in the workplace:  The employer did not complete the OSHA 300-A form for workplace injuries and illnesses the occurred in 2007.    f) On or about October 12, 2011, in the workplace:  The employer did not complete the OSHA 300-A form for workplace injuries and illnesses the occurred in 2006.      Abatement certification is required for this item.
