Workers were exposed to potential fall hazards from heights that were measured to be (front) 23.0 feet and (rear) greater than 25.0 feet in the rear in that a fall protection system was not properly utilized.    The Contract Direct Roofing Company was previously cited for a violation of this Occupational Safety and Health standard 1926.(b)(1) which was contained in OSHA inspection number 313172496, citation number 1, item number, 1 issued on May 14, 2009 and;    was cited for a violation of this Occupational Safety and Health standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 312564180, citation number 1, item number 2, issued on October 29, 2008.
