29 CFR 1904.7(b)(5): The employer did not enter a check mark in the box for cases where the employee received medical treatment but was not transferred or restricted. On or about November 22, 2011, the employer did not correctly classify the following workplace injuries and/or ilnesses on the OSHA 300 Log for the calendar year 2011. a) On or about May 19, 2011 a recordable injury occurred when an employee received a puncture to the right knee from a nail, as the employee was kneeling to adjust the six inch vac hose inside a cooling tower, resulting in a hospital visit and treatment beyond first aid. The employee returned to work the next day, but quit and took a job with another company in another state. The employer did not mark Column J on the OSHA 300 log for "Other" recordable injury.
