29 CFR 1910.134(f)(5):  The employer did not ensure that the fit test was administered using an OSHA-accepted QLFT or QNFT protocol:    Excel Environmental Inc, at 2480 S Clermont Street, Denver, CO: On 11/18/11 and preceding, Excel Environmental Inc did not ensure that the fit test was administered using an OSHA-accepted QLFT or QNFT protocol.  Employees who were required to wear full face or half mask tight fitting air purifying respiratory protection were qualitatively fit tested using irritant smoke.  The employer did not follow the QLFT protocol in Appendix A, in that each exercise was performed for approximately twenty to thirty seconds.  This condition potentially exposed employees to a respiratory hazard.

29 CFR 1926.1101(f)(5)(ii):  The employer failed to notify affected employees in writing of the results of exposure monitoring either individually or by posting at a centrally located place accessible to the employees:    Excel Environmental Inc, at 2480 S Clermont Street, Denver, CO: On 11/18/11 and preceding, Excel Environmental Inc failed to notify affected employees in writing of the results of exposure monitoring either individually or by posting at a centrally located place accessible to the employees.  Excel Environmental Inc removed drywall containing surface texture, floor tile, and mastic in the northwest wing of Clermont Park.  The surface texture and floor tile contained chrysotile asbestos.  The work was Class I and II asbestos work.  The employer performed personal air sampling of workers multiple days during the approximate two week activity.  The employer posted the results of exposure monitoring for 11/2/11, but did not post subsequent results.  The employer failed to notify affected employees in writing of the results of exposure monitoring for the period 11/3/11 through 11/16/11.
