29 CFR 1926.1053(b)(1): The side rails of portable ladders used for access to upper landing surfaces did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used, nor was a grasping device provided:    a) Robert Harmon's Roofing, Perrysburg, Ohio:  On or about November 21, 2011, at a new residential construction site located on River Ridge Way in Perrysburg, Ohio, the employer did not ensure that the side rails of an extension ladder were extended at least three feet above the landing.  Employees were exposed to falls while using the ladder to access the working level to perform roofing activities.    Robert Harmon's Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA Inspection Number 311603492, Citation Number 1, Item Number 1 and was affirmed as a final order on June 30, 2008, with respect to a workplace located at 13319 Roachton Road, Perrysburg, Ohio.
