OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire hazards created by an oil burning heater which was modified by the company to accept a fuel source which was not approved by the manufacturer. a. G2 Revolution, LLC located at 200 Stanford Parkway in Findlay, Ohio: On or about November 18, 2011, the employer did not ensure that the oil fired heater located on the south side of the facility was burning fuel approved by the manufacturer. The employer was burning an alcohol based hand sanitizer with a flash point of 77 degrees fahrenheit rather than a manufacturer approved fuel with a flash point of at least 140 degrees fahrenheit. The alcohol based hand sanitizer caught fire inside of the heater and then dripped out of the furnace housing and ignited a plastic 275 gallon tote of alcohol based hand sanitizer sitting under the heater. The fire spread to adjacent combustible materials stored around the heater and subsequently caught the entire facility on fire. Abatement Note: Among other methods, one feasible and acceptable abatement method to correct this hazard is to comply with the manufactures Installation and Operation Manual (October 2011) Waste-Oil Furnace Models: EL-200H, but not limited to: a) Ensure fuels used by the heating system have flash points between 140 degrees F and 400 degrees F. b) Ensure the heater is being installed and operated according to the Installation and Operation Manual.

There was approximately 3,000 gallons of hand sanitizer on pallets located in one area near the north end of the facility.

The employer had a sprinkler system installed in the building but the system was not hooked up to a water source.
