29 CFR 1910.134(c)(1): Where respirators were necessary to protect the health of the employee, or whenever required, the employer did not establish and implement a written respiratory protection program with worksite-specific procedures as required by 1910.134 (c)(1)(i-ix): a) On or about December 1, 2011, at the above located workplace, an employee working in the production area was required to wear full face air purifying respirator. The employer's written respiratory protection program did not include the following elements (as required by 1910.134 (c)(1)(i-ix)): (i) Worksite-specific procedures for respirator selection, (ii) Worksite-specific procedures for medical evaluations for users, (iii) Worksite-specific exposure monitoring results, (iv) Worksite-specific respirator canister change schedule, (v) Worksite-specific procedures for regularly evaluating the effectiveness of the program
